When someone emails out from the exchange server to places like gmail, yahoo, hotmail they never come thru. If said person emails the exchange user and the exchange user replies to it, then said person gets that email.

    How come?

you sure its not 550 5.7.1 "unable to relay"?

Thats pretty common among exchange policies

Edit: A lot of times, the problem is that exchange doesn't have a resolver(not dns), mx records are whack, SPF records

Add a resolver(your ISPs) to the exchange server(make it the first resolver), see if that helps?
