since i've been at a desk job for roughly 13 years now (holy shit i didn't know 31 is old :() i've developed an awesome slouch while sitting at my desk. In the past year i've been attempting to correct my posture but it actually starts to hurt after awhile. are there any exercises/stretches i can do to help this out

What you do in the gym can't fix what you do the other 23 hours of the day. Correct your posture whenever you catch yourself in poor position, and avoid sitting when possible. I have my desk set up so i can stand at my computer instead of sitting.

Ceaze is right, but correcting it outside of the gym can be easier if you make the necessary muscles stronger, so in essence, it takes effort all the time, but the amount of effort you have to put in can be lessened if you train properly.

Pushups, scap pushups, face-pulls, overhead shrugs, shoulder dislocations, etc all helped me have more strength in my postural muscles, which makes it much easier to keep them in a good position all day. Even lateral raises and other upper body stuff helped, because once I started using good posture during those exercises, it taught me how to move around all day while keeping my shoulders back and down.
