Last night, I noticed my computer was freezing every few minutes. I decided to reboot, and Windows told me that my NTOSKRNL.EXE was corrupted. I deleted it, copied it from another machine over the network, and sure enough, more files were corrupted. I was finally able to do a CHKDSK /F on the drive to fix the errors and my computer booted. This morning, the errors re-appeared. I fixed them and all throughout the day my computer has been fine, until now. It has started to freeze again, so I do a CHKDSK to see if anything is wrong and I see this:
What should I do? Would a format help, or is it a problem with the physical drive?

Drive fucked. Buy a new drive and try to clone it. Ghost has an option to force the cloning even if parts of the disk are unreadable. You might still need to reload the OS but hopefully you'll get your data off.

I'm actually still running off the drive right now. Doesn't seem to be freezing anymore, but I'm backing up my stuff like crazy.
