if toeshoes somehow got the motivation to become active again, she would gain lean muscle mass and lose fat, without steroids

    it's not rocket science

You can but it is very difficult unless you are:
    grossly obese
    a complete newbie

You won't find many people who have trained a long time who can add muscle while losing fat. Maybe a few genetic freaks or people following a cyclical diet like UD2.

How does someone gain muscle when they are in a calorie deficit?

I thought this was the reason bodybuilders "bulked"
