Its a military police pistol. is 9x18 closer to .380 than it is to 9mm luger?
I own one, pinches me whenever i shoot it, but a couple of my friends have been able to shoot it without pain, just depends on the size and shape of your hand.
.380 = 9x17
9mm Luger/Parabellum = 9x19
So, 9x18 Makarov is right in the middle of the two. It's not the easiest round to find. Most, if not all of the online ammo stores sell it, but the cheap stuff is steel cased.
I think it would make a nice BUG, but nothing more, unless you are lucky enough to not get hurt by it, otherwise mine is been very reliable.
did you know that you can get regular grips for it rather than the molded finger grip it comes with. Would that help you not get pinched? any pictures of your next to something that shows the actual size of the pistol?

I like mine. Ammo isnt too bad to find. I snagged some brown bear ammo for $12/100 at a gun show. I bought 500 rounds.
I am probably going to buy one along with a M44 because they will only charge for 1 FFL transfer at my gun shop
It's not the grips, its the beavertail(?) section below the hammer, it's shaped with small hands in mind, so the recoil pushes really hard against the web of my hand between my thumb and forefinger.

Notice how the PA is smaller than the XD sub. The part below the hammer is the part I was talking about that pinches your hand.
